The UK Legal Framework for Medical Cannabis

In the UK, medical cannabis is a highly regulated treatment. It is not a first-line therapy and is only considered when other licensed medications have failed to provide adequate relief. Who Qualifies for Medical Cannabis?

To be eligible for a medical cannabis prescription in the UK, you must meet several key criteria. First, you must have a diagnosed medical condition for which there is evidence that medical cannabis could be an effective treatment. Second, you must have tried at least two conventional, licensed medications or treatments for your condition without sufficient success — this is the treatment-resistant pathway. Third, only a specialist consultant registered on the GMC specialist register can initiate a prescription; GPs cannot prescribe CBMPs. Fourth, certain health conditions or medications might make medical cannabis unsuitable, and your specialist will assess your overall health.

Commonly Treated Conditions

While the range of conditions treated with medical cannabis is expanding, some of the most common include: chronic pain conditions such as fibromyalgia, neuropathic pain, and chronic back pain; neurological conditions including epilepsy, multiple sclerosis, and Parkinson's disease; psychiatric conditions such as anxiety disorders, PTSD, and Tourette's syndrome; gastrointestinal conditions including Crohn's disease and ulcerative colitis; and palliative care to manage symptoms like nausea, pain, and appetite loss in cancer patients.

What Medical Records Do You Need?

Gathering comprehensive medical records before your assessment will significantly strengthen your application. You will typically need: a letter from your GP confirming your diagnosis and the treatments you have tried; any specialist letters or reports relevant to your condition; a list of all medications you have taken, including doses and dates; and details of any side effects or adverse reactions to previous treatments.

What Happens During Your Assessment?

Your initial consultation with a specialist clinician is a comprehensive medical assessment, not a simple prescription request. The clinician will review your medical history in detail, discuss your current symptoms and their impact on your quality of life, and assess whether a CBPM is clinically appropriate for your situation.

The consultation typically lasts 30 to 60 minutes and can be conducted via video call or in person. You will be asked about your diagnosis, previous treatments, current medications, lifestyle factors, and any concerns you have about medical cannabis. The clinician will also assess for any contraindications — such as a personal or family history of psychosis, or certain cardiovascular conditions — that might make CBMPs unsuitable.

If the clinician decides to prescribe, they will recommend a specific product, dose, and administration route. They will also explain what to expect in the first weeks of treatment and when your first follow-up appointment will be.

What If You Are Not Eligible?

Not every patient who applies for a CBMP prescription will be found eligible. If the clinician determines that medical cannabis is not appropriate for your situation, they will explain their reasoning and may suggest alternative treatments or pathways.

Common reasons for ineligibility include: insufficient evidence of treatment resistance (not having tried enough conventional treatments), contraindications such as a history of psychosis or severe cardiovascular disease, age restrictions (CBMPs are generally not prescribed to patients under 18 except in exceptional circumstances), and conditions for which there is insufficient evidence to support CBMP use.
